PC ISP v4.0 Overview At the link below, you will find a video overview of the coming updates to Virginia's Person-Centered Individual Support Plan (PC ISP). Primary updates this year will move the Risk Awareness Tool (RAT) into the ISP to: place the tool into the SC workflow create the opportunity to reduce redundancy and streamline ISP elements, and automate identified risk factors within the ISP We will be holding trainings this summer prior to launching the ISP, which will be announced through this listserv. We hope this provides you with a basic understanding of core ISP updates that will be in v.4.0 of the PC ISP.
