error after scanner console pushes dicom to dcm4chee

Hi experts,

So after pushing images to dcm4chee from the scanner console, I get a lot of these errors "TransactionRolledbackLocalException".
I've attached the log - it occurs after the usual C-STORE-RQ success indicating data has been successfully transfered to dcm4che

Anyone had similar experience?
