MPPS2ORM is not populating Insitution Name

Hi,

I need to include the institution name & ordering physician in the ORM being generated. I have configured the mpps2orm.xsl with the DICOM tags but the generated message doesn't have this information.

Attached is the mpps2orm.xsl.

Can someone help me here?
