Dcm4chee Forward is one quick solution, but this is quite bulky to install dcm4chee a DICOM Router. I suggest you should give a try at Orthanc, this is very light and works as windows service. has a good autoforward facility via DICOM or using HTTP.
There are instance when DICOM Routers are subjected to proxy server where DICOM communication will fail, in that case Orthanc can use HTTP to commuicate to the server. If you need HTTP based transfer, then you have to install Orthance on both sides. If you need further please check this
