I'm wondering why the Y only glitch is happening.
Please let me know if it's a sensor or interface issue.

It's a sensor design issue. There's some internal corner cases where a pixel can deselect its REQ signal in the time window between the readout circuits seeing the REQ on the Y axis (which is checked first) and generating the Y address, and then checking
the X axis and then finding nothing there.
