Is it possible to set access restrictions on individual files in unpublished datasets?
Right now (4.2.2.), it appears that users who have been granted permissions on downloading individual files need to have at least Member or higher level of access to the dataset to download anything, and once they do, they can download all files regardless of access restriction level.
In other words, is it the case that:
- File permissions only come into play once a dataset is published, but for unpublished datasets, they can be set but are quietly ignored, and only the user's role on the dataset level matters
- Likewise, "file downloader" role is only taken into account once a dataset is published?

You're right that as of 4.2.1 the "File Downloader" role/permission only takes effect once the dataset is published. That's what https://github.com/IQSS/dataverse/issues/2645 was about.
That said, you should able to assign this on a file by file basis.https://github.com/IQSS/dataverse/issues/2653 has some todo items about documenting each specific permission more.

We are planning on adding in this functionality during 2016: https://github.com/IQSS/dataverse/issues/1012 if that is close to what you were hoping to be able to do.
Yes would be the answer to both items you mention in the "In other words..." section of your post.
Are you hoping to have restricted unpublished files that someone can access without having been assigned a role for the unpublished dataset? If so, that is not currently possible in Dataverse. The issue is that the file is within the dataset and there is no way to actually view the file without navigating to the page for the dataset. Therefore, if the user has no access to the dataset page, there is no way for them to get to the file via the UI.
Does this make sense?
