Regarding self-administered questionnaire

I have a query regarding the self-administered questionnaire. How can it get registered in one's name? Whether getting it published in a research paper is an appropriate way or some other way will be apt?

I'll be grateful if someone can guide me regarding the whole process.

 Mam publishing a paper with developed scale in enough to claim your right on the scale.

You would be offered copy right form while getting the paper published.The form you sign would cover the relevant issues.

Copyright is the only IPR that does not require registration.

Merely by virtue of its publication in any public domain, it is protected under Copyright Act 

If someone steals your ideas and work, you've to file a case.

If you protect your work apriori then register your work with the Copyright office
