Those who are conducting online surveys may follow certain guidelines. I found the first page of an online survey conducted by Ingrid from the University of Akron to be very helpful.

Some students include the emails of those who have passed away which to me appears to be quite disrespectful to departed souls. I feel that the research supervisors need to be more cautious and inform the students about this.

Title of Study: Personal Growth Self-Efficacy
Introduction: You are invited to participate in a research project being conducted by Dr. Ingrid Weigold (a faculty member in the Department of Psychology at The University of Akron), Dr. Arne Weigold (a faculty member in the Department of Psychological Science at Kent State University), and their research assistants.
Purpose: The purpose of this project is to gather information about personal growth to inform the initial development of items for a personal growth self-efficacy measure. We would like to get information from approximately 300-400 participants.
Procedures: If you agree to participate, you will be asked to answer two open-ended questions about personal growth and five demographics questions. This will take you about 5 minutes.
Exclusion: You must be at least 18 years old to participate in this research study.
Risks and Discomforts: No adverse events are expected beyond those encountered in daily life.
Benefits: You will receive no direct benefit from your participation in this study, but your participation may help us to develop initial items for a measure of personal growth self-efficacy.
Right to refuse or withdraw: Participating in this study is completely voluntary. You can quit at any time and you won’t lose anything.
Anonymous Data Collection: We will not ask you your name. All data will be kept confidential and only the researchers will have access to the data. However, following guidelines in the United States, other researchers may request our full data set (including your answers), which we would share. Participants will not be individually identified in any publication or presentation of the research results. Only aggregate data will be used.
Confidentiality of records: Your answers will be put in a computer file by number, without your name. The raw data will be kept for no less than 5 years and destroyed after that time in accordance with APA guidelines.
