DART License From

Hi all,

I'm looking to get a DART License but I find the form a little confussing and want to clarify the meaning of some fields.

In the account information you have 'Organism' as a field. Is this Organisation? 

In the license information you have head's name, first name, and function. I don't understand the difference between name and first name. Is name meant to be the surname? And function, is that meant to be position (like founder, Head of school, ect)?
