The Y chromosome is an isochromosome Composed of long arms of Y.
What is the sample type? Is it from bone marrow or peripheral blood?

Hello! At first glance this looks like an i(Y)(q10) although in a couple of pictures it looks like an idic(Y)(pter). Is it likely that there are more than one cell lines where the abnormal Y has different forms? The referral reason would help a lot-please share with us. Also, C-banding (for Yqh and number of centromeres) as well as  FISH with Y chromosome probes or arrayCGH would help to clarify the derivative Y. If you are to report it as it is without any more tests you should report it as a derivative chromosome. Also, try to find longer metaphases if possible. If that's a bone marrow sample do a peripheral blood karyotype for longer chromosomes.

That was my first thought too, isoYq. What is the patient sex and referral reason. 
Have you FISHed with a Y chromosome panel (X/Y subtelomeres, centromere and Yq probes, SRY)?

Its probably isochromosome , we have  a similar case years ago the patient came to the lab from infertility clinic .
We did FiSh analysis using the cytocell probe

Hi dear colleagues,
 
I studied a similar case (dicentric Y chromosome in a girl referred for testicular dysgenesis) many years ago and found a difference between heterochromatic fluorescent regions (QFQ-banding) in both their sizes and mitotic condensation patterns depending on the proximity to active/nonactive centromere (Kovaleva NV, Yakovlev AF. Morphology and mitotic condensation features of the human Y-chromosomes with structural rearrangements. Tsitologiia 1986, 28(12):1322-8)Unfortunately the girl's father was unavailable.
 
Since then, no such studied were carried out...
