Borang SPR 765 is on page 25 of the Buku Panduan Ejen Tempat Mengundi, May 2012. Download from attachment here: http://bit.ly/T-PAGPACABA1

• SPR 757 Catatan Bilangan Pengesahan Kertas Undi,
• and 764 Penyata Awah Pengiraan Undi

has always been used without question. It does not appear explicitly in the COE. 

I suppose SPR 757 arises from COE R25 (3)(b) and SPR 764 arises from COE R25 (13)
