How can I activate Contribution PlugIn?

I installed Contribution plugin and selected my homepage with 'Contribute an Item' on dashboard > Navigation.
When I access homepage, Contribute function displayed below Google Map Box, but didn't work.

1. How to activate contribution plugin on Contribute page(or simple page)?

2. I want to use contribution functionality on every story.
I will make a description and some people who has story or photo can upload text or image files.

The Curatescape theme does not officially support the Contribution plugin at this time, so you'll need to create your own template.
