Conversion of public company to private company

I need to file MGT-14 for conversion of public to pvt co. now the problem is for this we need to pass 3 resolution for change of name from public to pvt , alteration of memorandum and alteration of articles but the form is saying you cannot select any other purpose if the selected purpose is change in memorandum and article. what to do? Someone please help with this.

Even if I am choosing to pass only 2 resolution that is for alteration in MOA and AOA, while submitting it says to proceed to EMOA and EAOA and after proceeding I am redirecting to this page as attached here and after selecting ok again it tells to proceed to EMOA.
