Anyone can Share format for "Declaration with regard to fulfilment of fit and proper person criteria, as per sub-rule (3) of Rule 3B, by all Promoters and Directors of the Company"

Required for filling of NDH-4

I have been applying for RUN for Reservation of name for Nidhi Company, it being under processing for long and required resubmission again and again without any specific reason, even attached declaration as required. 

Can any one help me out specifically to sought it out on immediate basis.

Is this as per the form on V3? Last i checked auditor certificate is no more required because it has been incorporated in the form itself. It now requires the auditors DSC
