PRODUCER COMPANY REGISTRATION

The company was an agriculture producer company with 10 farmers. The Form spice part B was sent for resubmission with following remarks: 

Submit valid proof of the proposed members being producers as per Section 581C(1) read with Section 581A(k) and 581A(j) of the Companies Act, 1956. Such a proof submitted should be subject to verification of records from the concerned land / revenue department / Government authority / administration.

I had attached certificate from Agriculture officer for all 10 subscribers in the atatched format. 

Could anybody help me giving correct format of producer certiifcate
