INC 24 Query - Affidavit

Dear All,

Can anybody share Format of Affidavit to be filed INC 24. 

I got a query stating "File an Affidavit signed by at least 2 Directors stating whether there is any default on the part of the Company in filing statutory returns or any failure to pay or repay matured deposits or debentures or interest thereon as required under Rule 29 of the Companies (Incorporation) Rules, 2014"

It's Normal declaration 

Just drafting a declaration with mentioned above words which was raised in Resubmission
