CS 15 Mth Apprenticeship Contract / Agreement

Dear All,

I am looking for soft copy of contract or agreement letter for 15 months CS Training.

If possible can anyone please provide me the soft copy of the agreement letter. And please provide me the soft copy other than the one provided in the Institute portal(i.e. the scanned copy of the agreement letter).
