format required -PAN undertaking of foreign subscriber

Hi,

While registering a private limited company with a foreign director we received the following re-submission from MCA: 

PAN undertaking of foreign subscriber or foreign authorized representative should be attached pursuant to General
circular 12/2014 dated 22.05.2014.

Can anyone plz forward the format and does this require apostle again? Time left is very less to incorporate the Co.
