Should I attach Identity and Address Proof Documents of both the Assignor and Assignee or those documents are NOT required at all ????

I have attached ONLY Power of Attorney and Trademark Assignment Agreement.  Would that be sufficient ????.

Affidavit declaring what ???.  In my case Assignor is the Mother and Assignee is the Son.  So will there be anything to declare at all in my case Sir ???
