Is it necessary to execute sh 4 in case of gifting of shares in demat form ?
Acc to me, gift deed and DIS will suffice the purpose.

For demat, SH 4 is not required.

What about execution of a gift deed is necessary along with dis slip is sufficient?

Yes gift deed is compulsory but in demat transaction we are required to select as gift while transferring the shares from one demat account to another

Thanks, so in short only dis slip and gift deed is required? sh-4 is not required to execute.
