Format of letter for obtaining NOC from RBI

Can anybody provide me the format of application to RBI for obtaining NOC for name change of an NBFC and list of  documents to be attached with the application?

Dear Member,
In case of NOC from RBI for name change, you have to just send request letter on the letter head of the company along with board resolution passed in this regard to RBI, and has to liaison with RBI officers for getting NOC. Before giving NOC, RBI wants that all previous compliance and replies of letter if any issued by RBI should be completed.
