CHG-4 beyond 300 days -condonation of delay

Dear All,

For condonation of delay application (CHG-8) w.r.t CHG-4 beyond 300 days,  besides documentary evidence of satisfaction and copy of BR , are we required to attach any other document?

Normally how long does it normally take to get the RD order? The Company is Mumbai based .
