Query regarding Duplicate Share Certificates

Dear all,

 

According to the requirement of provisions of Companies Act, 1956, in case of issue of duplicate share cerificate ( when original is lost) , is it necessary that shareholder has to execute an affidavit and an indemnity bond in favour of the company and a notice of such loss has to published in the news paper .

 

Is it Obligatory to do so.

If yes, then please provide us the fomat for affadavit & an indemnity
