format of complaint against company before ROC

Can anyone provide me a format of complaint against a pvt. ltd company for non compliance of Companies Act provisions before ROC ?

we want to inform to roc that a pvt. ltd. has made some non compliance under companies act.

I think there is no format for this, you can just write it to ROC in your own language.

U can file a compliant through Investor's Grievance on MCA21 website
