Require guidance for conversion of proprietorship concern to private limited company

Respected Sir/Madam

I have certain questions with regard to the above conversion:

1. Do we need to attach any document while applying for name reservation?

2. While preparing the incorporation forms, do we need to attach a Business Takeover Agreement? If yes, who will be the parties to the agreement, since the Company is not yet incorporated?

3. Do we require a valuation report for transferring the assets and liabilities from the proprietorship concern to the Company?

I have attached noc from proprietor for conversion of proprietorship to copy along with general statement for takeover by company for name reservation and proof of existing business with all registrations. both name and company were approved consecutively.
