You can pass the resolution as follows:

Mention the details of bank account and reasons for shifting of account from the current branch in the preamble...

“RESOLVED THAT consent of the Board of Directors be and is hereby accorded to shift the current account number ___________of the company from State Bank of India, Deccan Gymkhana Pune Branch to State Bank of India, Chinchwad, Pune Branch"
