Buy back of Shares & SH 11 attachment format

Dear all, 

Has anyone completed buyback of a private company. Please share the forms filed with ROC. 

Also does anyone have the format of the attachment to be done in E-form SH 11. I have the format of SH-15 but i want the format of Description of securities to be bought back & particulars of the holders before buy back.

Are there any other documents that needs to be attach in SH 8, SH 9 other than the compulsory attachments.

Please guide me.
