Without preparing minutes and other documents, how can one ensure that meeting was actually held.

As per my view, agenda and minutes are to be prepared by the Company Secretary, but the same should not be minuted. It should just be placed in the meeting of Board (Minutes of Board Meeting) i.e how the evaluation is made in the meeting of Independent Director.

And there is no such specified format of that agenda and minutes of Independent Directors meeting.
