STK-2 Strike Off -Resubmission

Dear Professionals,

We have got a resubmission remark from MCA stating:

"Bank account closure statement or declaration for bank closure or not opening of bank account not furnished."

We already submitted in the affidavit and indemnity bond that the account is closed.

SBI bank is not giving any closure letter. They have given a bank statement where last column line they have mentioned " Close Account -Cash".

Since MCA do not offer any additional chance of resubmission, I request any of you to please share the format of declaration for bank closure and does it need to be printed on stamp paper and notarized?
