Format of documents required for condonation of delay

Hi

Anyone has format of application and resolution required for filling condonation of delay? Please share.

The company is being converted from a private limited to public limited company and there has been a delay in filling the form for the same.

Can anyone help me with the draft documents for filling an application for condonation of delay in filing of Form INC-28. The form is being filled 15 days beyond the date of filling of special resolution.
