PAS 3 filing for allotment of CCPS through private placement

Hi Members,

We have made offer for 4167 CCPS (Compulsorily Convertible preference shares)  through private placement.

I have few query w.r.t. the subject line mentioned above:-

1.  What should be type of allotment ?: a) Private placement or b) Preferential issue ( this i need to select in PAS 3)

2.  Company is making allotment in 2 parts (say 2083 and 2084), so in post issue share capital structure-

What should i mention in Issued, Subscribed and paid up capital of CCPS

Issued 4167 or 2083 

Subscribed4167 or 2083

paid up 2083

3. I am unable to mention MGT 14 SRN in PAS 3 as it is showing in-active (non editable). 

4.  Is it mandatory to attached PAS 4 and Pas 5 in eFORM PAS -3?
