IEPF form- Annexure required BY RTA

Dear

I have applied IEPF form and accordingly RTA & company executive called for various document but some document as per format required and they have not given format for that document. Document list is as follow

1.Indemnity Bond – FOR CLAIMING SHARES – On Non Judicial Stamp paper on Rs.100/-

                                          FOR CLAIMING DIVIDEND – On Plain paper

2. Advance Stamped receipt (original) with signature of claimant and two witnesses. (Format is given at Annexure I)

3.  SIGNATURE DULY ATTESTED FROM BANK IN ATTACHED FORMAT AND AFFIDAVIT REQUIRED FOR CHANGE OF SIGNATURE

Please provide document format as per aboe list

After filing IEPF- 5 form on MCA portal, you have to download Indemnity Bond, Iepf-5 form and Acknowledgement receipt from TRACK IEPF SRN by entering IEPF CLAIM SRN. 

advance receipt has been removed by IEPF authority so it is no longer required. 

For signature verification you can download form ISR-2 from the google.
