You have to prepare a Trust Deed on Stamp Paper and get it registered with Registrar alongwith the ID proofs and Photos of Trustees. The Principal Trustee and two witnesses have to be personally present before the registrar for registration of Trust.

The application formats differ in various states.
Require those.

Recently I had got done in Delhi & Mumbai, some part of the documentation differed.

Dear Reema,

I told you the procedure which I have personally followed for registeration of Trust in Delhi. So far as other states are concerned, we definitely have to coordinate with the respective Registrar for registeration of Trust Deed.

Further, as far as my knowledge, the application form required for registeration is a just plain application giving details of particulars of Trust Property and source of income, if any alongwith ID proofs of all trustees and Address proof of trust. You may also refer the Rajasthan Rules for registeration of trust for the same.
