CHG-8 Condonation of Delay for charge satisfaction

Hello Everyone,

Anyone has recently filed Form CHG-8  for condonation of delay for satisfaction of charge?

Do we have to attach the petition and affidavits as there is no attachment mentioned in the form.

Please guide me.
