Trade Marks Agent Application - Character Certificate

Can any one share Character certificate format and relevant documents to be provided in reply to letter received by Trademarks Department after filing form TM-G for registration as a Trade Marks Agent .

If tM is objected and reply is sent but still today no reply form them What to do in  these case??
