I do not understand your requirements. However, I have assumed that you ask for soliciting investment by the investee company ( public ltd company - unlisted ) to the investor company ( listed company).
It sounds like private placement or private equity by an unlisted public limited company, which is governed by Rules formed thereunder Companies Act, 1956- Issue of Shares by unlisted public limited companies.
In my opinion, there is no prescribed share application form except expression of interest by the investor company agreeing to take such number of share for such price with such terms and conditions as deem fit duly signed by the competent person holding power duly authorised by the Board.
Allotment of shares up on receipt of consideration is to be done through board resolution of the investee company.
