MOA & AOA - SUBSCRIBERS SHEET

Can someone please forward me the latest subscribers sheet of MOA & AOA of latest companies Act, 2013.

I would also like to know should all the affidavits should be get notarised or not.

I think photo of every subscriber is compulsory now. so, attached after subscribers page one more page of photo of each and every subscriber.

and in Witness Coloum write the below mentioned statement:

"I witness to subscriber who have subscribed and signed in my presence, further i have verified their Identity Details for further Identification & satisfaction myself of their identity particular as filed in"

no photo is not required on sub sheet.. form inc-10 is serving this purpose..

Photo of every subscriber is compulsory in both MOA & AOA  subscribers sheet, If Photo is not affixed ROC is rejecting the incorporation forms.

It clearly says about the requirement of the Photograph.

As per Rule 16(1) (a) of chapter 2 say to affix the photos in MOA & AOA of the Subscriber. Because of not affixing the photo my incorporation forms was put re-submission, after affixing  the photo only it was approved.

Yes ROC, Bangalore will insist to mandatory write in your own handwriting in subscription sheet by the subscriber to MOA & AOA. If the details are printed, then they will reject the forms.

It is mentioned in the Incorporation Rules as well to attach the photo of the subscribers in Para 16 (1) (a). Bangalore ROC also sometimes asks for it, so does ROC Chennai. But ROC kolkata has not come up with the requirement yet. However, it is advisable to attach the photos while uploading since  the chances of re-submission have become really less.
