Request for emergency contact and medical info

Hello.

 

You have received this email because you are a member of the Crystal Palace Brass Band (Main Band) distribution list.

 

We're asking for emergency contact details for all members of the band, plus any special medical needs  - so we can make sure everyone is safe while taking part in rehearsals and gigs.  If you are a parent of a young member, please fill in on their behalf (using their name).  It is up to you if you want to provide details of medical conditions - this is just for information we might need to know in case of emergency and won't be shared outside the band leadership team.  

 

We have already have collecting this information for Progress Band,

 

Any information collected will be treated in confidence and will be held securely.
