Attached you will find a PDF of our CM MMX gate waiver.  If you print this and fill it out at home and bring it with you to the evet your gate experience will go quicker.  

You must sign the document in-person but you can have everything else filled out ahead of time.  Please mark the dates appropriately with the day you are arriving at the event.
