vecuronium and feeding

Hi, we have been using this more than usual lately. Is it OK to Tube feed with vec if HOB is up and there are no large gastric residuals? thanks, Karen RD

Vecuronium falls into the category of non-depolarizing muscle blockers (NMB) and includes others such as pancuronium, rocuronium, atricurium etc. These agents affect only skeletal muscle and do not affect smooth muscle and therefore do not affect GI tract motility. EN can be used safely in these patients and the usual strategies should be used to optimize the benefits and minimize the risks of EN.

Hi Dr. Drover: would being on an oscillating vent in and of itself be a reason not to TF? My patient is still on vec but HOB is now up to 30 degrees. They started TPN because they said that they can't enterally feed while on paralytics on an oscillating vent. Thanks, Karen

Absolutely not! The oscillator or paralysis either alone or in combination are not a contraindication to EN. Good luck.
