Stroke Clients upon Admission

Good morning!

I'm looking into validated screening tools used in the initial nutritional screening of a stroke client upon admission.  If your facility currently uses one, please send it my way.  

I would also be interested in how patients are followed up once the screening has been completed.

I'm happy to share my findings with this group following my research!
