Version 0.3.0 released

CppWAMP v0.3.0 has been released. This version include refinements to the API, particularly with registration/subscription management, as well as how results/errors are returned from an RPC. The minimum required Boost version has been increased to 1.58, because it now includes Boost.Endian. The tests and examples now use raw socket handshaking, which is now supported on Crossbar.

I'm quite happy with the current API, so hopefully there won't be many more breaking changes.
