Employee Reviews / Form

[Mojo]

Hello,  I'm approaching the first year review of our community manager at Mojo Coworking and I was wondering if anyone out there has put together an employee review form that I might use as a starting point for creating one for Mojo?  I started searching through Google for a general review form, but figured it would be much better to use a coworking-based form if anyone has one.  If so, please reply and attach.  Thanks.
