2017 CLSA Membership Information

All members of CLSA should have received their annual membership invoice and forms to specify their preferred dates for their obligatory assignments.  I failed to include an important piece of instruction in my annual letter to the Club.

Included with your bill this year is a form for volunteering for Ohio State Parks.  This form is required for anyone doing work on our leased property.  It is required by the Ohio Department of Natural resources and gives worker’s compensation coverage.  If more than one family member will be working on State property, please copy a separate form for them to sign.  The previously signed forms all expire on December 29, 2016 and the new forms cover you for two years.

These forms should be returned with your membership dues.  Your annual renewal will not be considered complete unless this form is returned.  If you have already sent in your dues (and did not include the form), you can bring it to the Winter Dinner on January 21.  You can also mail it to Tim Gilliland at 990 Long Road, Xenia, OH 45385.
