New Form 100 will be available at the 1 Dec Council Meeting.

My Brothers All:
Supreme has distributed a new Form 100 to all Financial
Secretaries that are to be used IMMEDIATELY in place of the
present ones. Older versions of the form are not to be used
following receipt of the new form. All existing supplies are
to be destroyed. The new forms have the revision date of
10/11 following the form number 100-MD in the upper left
hand corner.
The major change in the Form 100-MD 10/11 is related to the
New Member Annuity offer for the applicant and his wife. The
interest rate quoted on previous revisions has changed and
is no longer printed on the reverse side of the Council
Copy. The product remains available to new members and their
spouses between the ages of 18 and 85. The interest rate can
be obtained by calling Supreme at 800-524-3611 and selecting
option 7. The new form also has a new paragraph added to the
back that better defines what a constitutes a practical
Catholic. The front of the form is essentially the same.
To repeat, all members are to begin using the new Form
110-MD 10/11 IMMEDIATELY. Destroy all existing supplies with
older revision dates.
