The PFS Look-Up Tool helps you find Medicare payment amounts for each code so you can calculate the patient coinsurance amount. The PFS gives the limiting charge for nonparticipating providers and suppliers who treat Medicare patients.

We adjust the PFS pricing amounts to reflect the variation in practice costs from area to area. Each Medicare payment locality has a geographic practice cost index (GPCI) for the 3 components of a procedure's relative value unit (like the RVUs for work, practice expense, and malpractice). We apply the GPCIs in the calculation of a fee schedule payment amount by multiplying the RVU for each component times the GPCI for that component.

This display tool is an aid for physicians and nonpractitioners looking for PFS payment rates. We make no warranties, expressed or implied, regarding errors or omissions and assume no legal liability or responsibility for loss or damage resulting from the use of information in the tool. For the official and definitive CMS PFS payment files, contact your MAC.

We look for those students we believe will thrive at USC. Our application process is designed to discover your individual story, so that we might see how you would take advantage of the many opportunities available at USC.

Like many highly selective universities, we conduct a comprehensive, holistic review of your application to consider academic and personal characteristics. We will review your performance in school, the rigor of your program, your writing skills and any test scores you choose to submit. We also consider personal qualities, as revealed in community involvement, leadership and achievements.

Teacher and counselor recommendations speak to your academic success and the contributions you might make to the classroom, the broader learning environment and the community at large. Your values will be reflected in those letters, as well as in your activity list.

We pay close attention to your personal narrative, background and circumstances, realizing that each student is unique and has something to offer. Your application will paint a picture of you and how you might contribute to the community, tradition and spirit that make USC the dynamic place it is.

USC students pursue ambitious intellectual and professional goals by studying across disciplines and taking advantage of the diversity of programs available. They are willing to venture outside their comfort zones. They are interested in the world, in other peoples and cultures, and enjoy examining important issues from a global perspective. USC students are unafraid to speak up in class to make others think or fight for a cause. They get involved by participating in student organizations and connecting with others. They seek to grow to their fullest potential, and they work to serve others in the community along the way.

Outside of mathematics, no specific curriculum is prescribed or required, though students offered admission typically pursue the most rigorous program available to them in English, science, social studies, foreign language and the arts. Students are expected to have earned a grade of C or better in at least three years of high school mathematics, including Advanced Algebra (Algebra II). Careful attention is paid to preparation for the intended major.

The Annual Security and Fire Safety Report includes Clery Act crime and fire statistics for the preceding three years for locations owned and or controlled by USC, the required policy disclosure statements and other important safety related information. A paper copy of the ASR is available on request made to Department of Public Safety Records by calling (213) 740-6000, by email to DPSRe...@dps.usc.edu or in person at DPS at 3667 South McClintock Avenue, Los Angeles, California 90089.

The admissions process at Brandeis is as comprehensive as it is selective. We consider your application holistically, but pay particular attention to three factors: academic qualifications, potential for social and academic success, as well as a demonstrated understanding of Brandeis' mission.

Above all else, we want to see that you've challenged yourself. To be considered for first-year admission, you should be enrolled in a college preparatory course of study or equivalent homeschool environment.

Most applicants have taken honors, advanced placement or international baccalaureate-level coursework when available at their school. There's no minimum GPA for admission to Brandeis, but more than three-quarters of our accepted students graduate in the top 20 percent of their high school class.

Academic ability is an important factor in our admissions process, but your life outside the classroom matters, too. During our review, we look for how your interests, activities and passions align with the university's mission: to prepare active citizens who care deeply about improving the world. No one student can encompass all the diverse and unique traits that make up our student body, but here are some of the qualities we consider:

Critical Thinking: Show us how you demonstrate creativity and curiosity. We look for evidence that you may delve deeper into subjects, projects or ideas or may not accept information at face value. We appreciate how you question, analyze, evaluate, invent, synthesize and seek out additional sources and opinions to expand your perspective.

Brandeis is a university dedicated to our founding values of inclusivity and justice. Established 75 years ago to address antisemitism and discrimination in higher education, we value the diverse array of backgrounds, beliefs, and experiences that our students bring to our community.

The quick-look assessment for the Higher Education Center at McAllen was made available online for review and feedback from Feb. 22 through Feb. 29, 2024. All McAllen employees and students received a survey for feedback, and on March 8, President Welsh hosted an in-person town hall in McAllen to discuss next steps.

Studies have shown that women (and nonbinary folks) are over-mentored, but under-sponsored. As Herminia Ibarra, professor of organizational behavior at INSEAD and coauthor of the HBR article Why Men Still Get More Promotions Than Women explains,

To sponsor someone is to feel on the hook to help get someone promoted. It is raising up the name of someone to help them get more opportunities to do visible, valuable work. It is not just giving advice and mentorship. Often, you can sponsor someone without them even knowing it (many of the people I sponsor have no idea I consider them sponsees).

Take a hard look at the daily communications you participate in: your work chat logs, the conversations you have with your manager and other managers, the process for figuring out who should fix a bug or work on a new project, and the process for making your work visible (like an architecture review, code review, launch calendar, etc.).

The Geographic Information System (GIS) now allows Colorado taxpayers to look up the specific sales tax rate for an individual address. The GIS not only shows state sales tax information, but it also includes sales tax information for counties, municipalities, and special taxation districts. This system allows for a complete tax rate to be determined so the correct tax is collected from customers in real-time.

For those who file sales taxes. This downloadable spreadsheet combines the information in the DR 1002 Sales and Use Tax Rates Document and information in the DR 0800 local jurisdiction codes for sales tax filing in one lookup tool. The spreadsheet includes local option exemptions for state-collected jurisdictions. A new spreadsheet will be provided here twice a year to show January through June and July through December rate changes. The web page will note the effective date of each spreadsheet and you should use the one that applies to the period for which you are filing. Under Colorado statutes, each spreadsheet will be available for three years.
