A cover sheet with the company/organization name (you can self-nominate), address, and the applicable award category (Climate Leader, Heritage Industry, Innovation, Rural Revitalization, or Non-Profit Organization).
A one- to two-page narrative detailing:
A description of the company/organization, its products and/or services, and years of operation
Commitment to employees, including the number of employees and average hourly wage (excluding management), and how the company prioritizes its employees through compensation packages, benefits, flexible work schedules, retirement plans, paid vacation, professional development, and other programs designed to help attract and retain talented employees.
Commitment to the community, including details on how the company serves the community it operates in, with information on volunteerism, philanthropy, workforce development/school partnerships, and other programs intended to help the local community.
Commitment to customers, describing how the company is committed to customer/constituent/member satisfaction and/or service excellence, including details on why the company produces "best in class" products of the highest value or how the organization is committed to its mission, supported by metrics and success stories.
The signature of the nominee.
At least three (3) letters of reference.
A completed Maine Revenue Services Interagency Authorization Form, which allows the Department of Economic & Community Development to check with Maine Revenue Services to confirm the business being nominated is in good standing with the State.
