You'll want to follow the instructions
here to get set up to read the eprom. This is for the Concord 4 and the orientation for the chip (at least for mine) is having the chip closest to the lever (furthest from the USB connector) with the orientation notch facing outward toward the lever. I know that this is different (actually opposite) from what is indicated on his page, but it's the only way that worked for me. Neither way blew up my chip but your mileage may vary and I will not be responsible for any damage caused as a result of these or any other suggestions.
The reason I'm posting this information is because I couldn't find it anywhere else. I was fortunate that a Dealer Code had not been set and so I was able to create one and then look it up on the EPROM
Upon reading the chip, I found nothing at the indicated addresses for the Concord 4, but I found the following for the Concord Express:
Master Code: 219C (8604) and 219D (8605)
Installer Code: 219E (8606) and 219F (8607)
Dealer Code: 2FF2 (12274) and 2FF3 (12275)
