Need a function that lowercases (or uppercases) the first letter of a string

Hi, does there exists a function in LaTeX that lower (or upper)-case
the first letter of a string ? I need it to automatically include some
informations in an index.

\MakeUppercase this? (no braces).

Is their any letter style in Latex where the whole letter would
be left justified? Alternately, how do we tweek the standard
letter style to allow left justification on everything?
