LaTeX questionnaire

I was going through ctan.org looking for a template/macro for a LaTeX
questionnaire to be used in a bachelor project.
I was, however, unsuccesful.

I was wondering if anyone had/knew of a template or something similar to be
used to generate a questionnaire in LaTeX?

I seem to remember somebody posting about a very nice questionnaire
package he had created and asking for feedback. I don't recall the name
of the package or the poster, but if you search Google Groups you might
find it. I don't think he called it a questionnaire -- you might need to
search on words like survey, poll, or form to find it. I thought he had
put it on CTAN (though I could be wrong about that), so you might also
try repeating your search there using terms other than questionnaire.
